By submitting this form, you agree that if your request is approved, you will assume "ownership" for the shifts you request, and will be responsible for either 1) working the shift or 2) finding a suitable replacement worker for your shift(s) should you be unable to fulfill your shift assignment.
If your shift assignment is approved, you will receive a confirmation of your schedule via e-mail no later than 5 pm on the Monday before the scheduled week. |
